This Diabetes Education Program is designed to empower individuals with diabetes, their families, and caregivers with the knowledge, skills, and confidence needed to effectively manage this chronic condition and lead healthier lives.

Register for the education portion only or add a 10-minute one-on-one session with Colleen to talk about your specific diabetes needs and concerns.

Collen Cook, PharmD, CDCES, BC-ADM, is a pharmacist (25+ years), Board Certified Advanced Diabetes Manager and Certified Diabetes Care and Education Specialist.  Born in Trinidad and Tobago, islands in the Caribbean, she was first exposed to Diabetes from her dad, aunts, uncles, friend and relatives.  Her passion, enegry and drive to enhance diabetes care grew and inspired her to develop these Diabetes classes to enhance Diabetes education in her community.  Her goal is to help navigate you through a path which enables you to overcome any challenges you face with your diabetes management.
